It's been a popular supplement for most of the last decade for these reasons, but for most of that time, there was only one type of HMB: HMB-Ca, which came in the form of a calcium salt. In recent years, a new version, HMB-FA or "free acid" has been promising even greater results.


HMB is a metabolite of the amino acid leucine, meaning that when we eat protein, a small percentage gets converted to HMB. However, you would need to consume 60 grams of leucine, or up to 750 grams of high-quality protein per day, to obtain an effective dose of HMB (3 grams per day) through your diet. Therefore, taking a supplemental form of HMB is ideal.


Recently, HMB free acid (HMB-FA) has been publicized as the superior form of HMB. Here's the reasoning: HMB-Ca generally has a slow rate of appearance in the bloodstream after ingestion, taking approximately 60-120 minutes to reach peak concentrations in the blood.[5]


On the other hand, when HMB is provided in its free acid form, the absorption rate is accelerated, resulting in peak HMB concentrations in the bloodstream approximately 30 minutes after ingestion.[5, 6, 7] Not only does HMB-FA appear more rapidly in the bloodstream compared to HMB-Ca, it also yields greater peak HMB concentrations. Lastly, an increased clearance rate was also observed with HMB-FA, presumably as a result of better uptake by the muscle.[8]


First and foremost, very few studies have directly compared the effects of HMB free acid versus HMB-Ca, making it difficult to determine if HMB-FA is truly a superior form of HMB. A recent study investigating the impact of HMB-Ca on muscle metabolism concluded that, despite the proposed differences in bioavailability, HMB-Ca provides a comparable anabolic and anti-catabolic effect compared to HMB-FA.[10]


When combined with resistance training, HMB free acid supplementation may lessen markers of muscle damage, augment immune and hormone responses, and enhance training-induced muscle mass and strength. HMB-FA supplementation may also improve markers of aerobic fitness when combined with HIIT training. Those are all compelling reasons to try it.


To date, the majority of researchers utilized supplementation of commercially available calcium salt form of HMB (HMB-Ca) [7,11,12,13] and research studies in this area reported conflicting results when supplemented with HMB-Ca during a resistance training intervention. However, a new free form of HMB (HMB-FA) has been shown to have a more rapid and higher bioavailability compared to HMB-Ca [14,15]. Fuller et al. [14] found that HMB-FA had a 25% greater clearance in the body and faster absorption when compared to HMB-Ca (36 vs. 131 min). These data suggest that acute supplementation with HMB-FA may be more effective than HMB-Ca for reducing muscle damage [16,17,18], inflammatory response [15,16] and anabolic hormones induction [10]. However, to our knowledge, only one study examined the effect of chronic supplementation of HMB-FA on strength and power adaptations following resistance training. Wilson et al. [8] investigated the effects of 12 weeks of HMB-FA supplementation on muscle mass, strength and power in resistance-trained men and found that HMB-FA induced greater gains in comparison to the placebo group. In addition, the creatine phosphokinase and cortisol responses to resistance training were lower for HMB-FA. The greater absorption rate and bioavailability of HMB-FA may provide greater benefits regarding its efficacy as a nutrient supplement used to enhance training adaptations [14,18], thus shortening the length of time needed between supplementation and exercise. This may potentially allow the athlete a shorter recovery time and an increase in performance in subsequent training session.


With review of hormonal changes (Table 2), these results suggest that HMB-FA may affect growth hormone/ insulin like growth factor 1 (GH/IGF-1) axis signaling; however, the effect on skeletal muscle protein synthesis requires more investigation. It is possible that the GH/IGF-1 axis signaling could require a large change in plasma HMB (i.e., HMB-FA) levels and it appears that chronic supplementation with free acid HMB form induced meaningful effects on GH/IGF-1 axis (very large to large training effects, Table 2) resulting in body mass increase and strength gain [27].

In a comprehensive review of HMB studies over the past two decades, the authors examine HMB efficacy. This review also examines why many newer studies, longer in length than some of the original studies and also conducted with HMB in the free acid form, are suggestive of stronger efficacy in both trained and untrained populations. The authors concluded that recent studies utilizing high-intensity training have demonstrated HMB efficacy in maintaining muscle mass and attenuating the inflammatory response. In particular these studies show improved efficacy of HMB free acid in trained athletes.


Elite infantry soldiers were supplemented with either HMB free acid (BetaTOR) or a placebo and underwent an advanced military training, which consisted of periods of restricted sleep and severe environmental stressors. Over the 23-day study the researchers saw a decrease in the insulin growth factor binding protein number 7 (IGFBP-7). The reduction in this protein indicated a reduced stress response, and warrants further investigation into the physiological role of HMB free acid (BetaTOR) in reducing stress during intense military training protocols.


Healthy men supplemented with HMB free acid (BetaTOR) (3 g/d) and underwent a six week resistance training program. BetaTOR supplementation improved the anabolic adaptations (GH/IGF-1) and decreased catabolic stress hormones (cortisol/ACTH). Similar to previous studies BetaTOR supplementation also improved strength and power during the training when compared to placebo-supplemented men.


This paper reviews the first nine studies conducted in humans with HMB free acid through 2016. The studies involved resistance training protocols and high intensity interval training (HIIT). HMB free acid was shown in all of the studies to improve outcomes by improving recovery, improving performance (aerobic metabolism and strength), increasing lean muscle mass and decreasing fat mass, and improving immune and endocrine response to intense exercise.
